Self-referral   Referral by a Health Care Professional

FREED is an adult service, but will accept adolescents over the age of 13 with parental consent. FREED will liaise closely with the GP if there are concerns about the client’s physical or mental wellbeing.

Please click on one of the links below to download the FREED Beeches Self-referral Form either in Microsoft Word or PDF format. Completed forms should be returned to the service at the above address or emailed to: info@freedbeeches.org.uk (Confidentiality is assured)

You will be invited to visit the centre and if you wish to access the service an appointment will be made for an assessment.

At the assessment you and the assessor will decide if the service is right for you and an appointment will be made for you to see the appropriate therapist.
