Self-referral   Referral by a Health Care Professional

Please click on one of the links below to download the FREED Beeches Self-referral Form either in Microsoft Word or PDF format. Completed forms should be returned to the service at the above address or email to: info@freedbeeches.org.uk (Confidentiality is assured)

You will be invited to visit the centre and if you wish to access the service an appointment will be made for an assessment.

At the assessment you and the assessor will decide if the service is right for you and an appointment will be made for you to see the appropriate therapist.

 

Health professional from the following areas:
Mansfield/Ashfield, Bassetlaw, Newark and North Nottinghamshire should refer through SPA/CAS. All patients who are being referred by a Health Professional in the above areas must do so by sending a referral to:

Any referrals from Health Professionals out of the North Nottinghamshire area should refer directly to FREED Beeches.

FREED is an adult service but will accept adolescents over the age of 15, with parental consent. FREED will liaise closely with the GP if there are concerns about the client’s physical or mental wellbeing.
